What was accomplished and performed last year and this new year is pretty much the same.

  There are projects/areas that I consider myself that I had performed above the expectation of my duties and
responsibilities.


1.  One of the most achievement of the year I had accomplished was my dedication to my job.  
Same as last year, I have dedicated myself to working hard to help different sites.   I had worked hard helping
many sites to accomplish fixing their problems the shortest possible time and sometimes during my CWS day
offs.   There were a number of tickets that needed urgent attention and provided the site the help they need. 
There was a time that we need to shutdown the Oakland FO Vista system due to Power Emergency Testing. I
have availed myself on shutting down the Vista System on Friday Night and had restarted the system on
weekend. We need to have a working Vista system the day before the developers and other users would
access it.
Our goal was simply to succeed in restarting the Vista System work for the users and especially the
developers to work with their Application developments.  There were times that we need to shutdown and
restart the Vista system, since they the Senior System Mgr was to install VMS patches. I have to avail myself
on Friday night or the weekend if we encounter some other problems.    With good  dedication to our job, I
availed myself during those times.  My regular duties are simply to ensure that the 3 clusters in our Oakland
Vista system are running, users are able to connect and access their development areas or build some test
scenarios before releasing to the production, examine/monitor if there were any major incidences of error,
and run the backup tapes for the ISC, Kernel and the system account.
There were times that I had to work voluntarily during my CWS days to help some unresolved tickets or/and
sites that my needed immediate feedback or resolution to proceed to do their job. Monday is usually the
busiest day of our workdays and there were times that there were some unresolved tickets and
customer/sites needed an immediate feedback or resolution to be able to proceed to the next level.  Helping
them by providing the information tru the email resolve the issues.  The resolution helped the site move
forward without delay vs. waiting for another Team member re-assessing the problem, and helping them.  Its
faster to provide my help since I had started working with the sites and don’t want them to wait the next day
or so.  We are a team and I would like to help in any little ways I can.  Had worked with site numerous times
about their Institution File issues and accomplished them during my CWS day off. 
One of the sites needed help during my CWS days off; You-ying is one of the customers that had problems
with their 200 series facility. Worked with her to ensure that they have their entries in their Institution File
were corrected and few entries needed special attention since they had to be registered in our National Dbase
immediately to process some of their data. Had worked with her closely with her Application Support person
to get extra help on getting their entries registered at their end locally. Had to work her since her
ADPAC/Regional support person was not available during the time she needs help. I was given access to their
site and assessed her account to ensure that she would be able to access the Institution File for Inquiry of
facilities she was working with.

2. Another most achievement of the year I had accomplished was in the area of National Institution file.
     Same as last year, I have been working diligently and closely with the Authority of National Institution File. 
They are the office of Financing Service Center in Austin which was designated the 104 office.    I had worked
with a number of 104 office staff especially with Naseem Majeed and Jay Kryk.  We were able to accomplish so
many request promptly and some were expedited since some facilities had opened up without following the

procedure of submitting the necessary papers.  To expedite their request and accomplish their goals before
opening (or facilities had opened and was not registered yet in our National Institution File), immediate
attention and quick turn around must be performed.   A number of these facilities had opened their facilities
and were not able to process patients since their *new* facility has not been existing and would need
intervention of the 104 FSC office and our team to get them going.   We were able to help and assist a number
of sites that needed their request expedited since they had already opened up or about to open in few days.  
Also, there was a site (Fayetteville Med Ctr) that having problems with their DSS project for quite sometime.
Their Institution File was configured correctly and has all the necessary entries. I had discovered that the
why the Site could not properly process their data. The site would need the entry (Stn 565GF) be entered in
File 40.8 (MEDICAL DIVISION FILE). Tested the DSS application and it was partially working. I also
discovered that they their File #389.9 (STATION NUMBER (SENSITIVE)) needed to put this entry Stn 565GF
in. These files are not our responsibility since Billing Application Support handles this. With the site’s re-
organization, no one was there to help them get this resolved. I requested that site to give me access to their
system and succeeded in fixing the problem. They have been processing data and applying a work around to
the problem. Now, they don’t have to use the work around and can process data smoothly. Susan is delighted
to hear that we had fixed her problem that’s been existing for quite sometimes.

3.  Another most achievement I had accomplish for the 2012-2013 period is taking the responsibility of the Junior
System Manager at Oakland Field Office. 
 
Ever since Chris Richardson and Fort Wally have retired, I was the only staff to maintain the Vista System in
Oakland.  With the help and guidance of senior System managers, Bob Schitone and Shane Miller and
previously Clause Jones. , I was able to handle some of the responsibilities.   I had worked with Bob Schitone
to maintain and System Administer it.   I was given the responsibilities of monitoring the System activities,
ensuring that its always running normally or the speed that is needed to run the developments routines,
providing access to new developers and de-activating retired users,  backing up the Oakland Vista system to
tapes and other *minor* maintenance the Vista system.   There were times that Vista System person who is
maintaining it must be in the physical location to fix the problem.  Certainly, someone has to be here in our
site to replace parts that is needed to get the Vista system working completed again.    I have been monitoring
the system CPU, any breaktru of unknown user/hackers, checking 1-3 days the Disk storage if they are
overwhelmingly increasing in size or had almost near Disk-Full level and ensuring that Taskmanager is
running properly.  I made sure that the journals are run daily and checked AXPNT if there are any
anomalies/errors.  Now, Bob Schitone was assigned to be the senior System Manager of Oakland Field Office,
and I work with  him closely to handle System Administration of the Oakland Vista System, resolve any
minor/major or unexpedted problems.
 
4.  Same as last year, had handled and resolved various tickets on time assigned to HSP team.  Had worked
with other Team  members (especially with not much exposure to Institution File, NPI Extract, New Person
Class and  few VDEF tickets)  to share what I know.  I had shared the knowledge and skills that are needed to
know on how to handle the intricacies National Institution File, NPI Extract, New Person Class and some of
the HL7 issues.   In another words, team player was adapted to work and help one another to share what I
know and vise versa.  

     Being specific, I had performed successfully and demonstrated satisfactory level in handling our
responsibilities related to following but not limited:
   
      -National Institution File Redesign
 
      -HL7 and related application interfacing with it (VDEF, LAB, VIC, etc)
 
      -Person Class
 
      -Fileman (few of them this year)
 
      -Mailman
 
      -Kernel (VA Alerts, Person Class, Device issues, etc)

      -Releasing patches in few applications (Mailman and HL7)
