---fix---solution---Institution File problem---
**dont change the address until you get an approval from the NPI team bcoz
of the billing App is expecting the old address
**you can ONLY change the .01 field -- NAME. But not the Address since it
has big impact to the Billing Dbase.
1. with Parent Assoc = NCSC
-Must be corrected at the site-
-Delete the Association
2. Extra NPI ; or extra NPI and extra Effective Date and time
-Must be corrected at the site-
-Use Edit, and use Field Eff Date and time, then put double Question mark to identify the line to be deleted
-Delete the Eff Date and Time; No need to delete the other line for NPI
3. Modifying Effective Date and time
-We dont this, unless we have a new NPI
-We leave the Eff Date and time alone. Whatever the old info, we leave it
4. Correcting Facility type
-Correct it in FORUM
-Populate it
-IF not transmitting, modify at the site
-Use the secret code of variable XUMF
5. Approval of activation ahead of time
-If FSC office has sent the approved Memo, just register and active but
-Make sure you put it as INACTIVE, and put in OFFICIAL VA NAME--Not active until xy-zy-zz
-then, populate it!
6. Entering CLIA;

Excerpt from E-mail sent to Ron Davies on 08/25
My guess is that most if not all of the alerts are false alarms. This
is a known problem that will be fixed in a patch (no date set yet).
One way to quickly check this is to run the VDEF Status display from
the VDEF Config menu. If the counter for Checked Out is at zero, then
all the alerts were false alarms. The definitive way would be to use
%G and look at the global node
^VDEFHL7(579.3,1,1,IEN,0) and look at piece 2. If it's "C", then the
record is stuck in the checked out status. If it's "P", then it's
been processed (false alarm alert). The record IEN is the number from
the alert message.
If the records actually are stuck in the "C"hecked Out status, then
that was caused by the VDEF message creation program halting with a
fatal M error. If this is the case, you will need to look in the
error trap for the associated errors. To know when to look, look at
piece 8 of the 0 node from the %G display. That's the date and time
the record was created in VDEF. If there were errors, the program
most likely will be a GMRAIA* program.
If the alerts were caused by genuinely checked out records that
errored out, there is nothing you can do since the reason is missing
data in the application's data file. Just delete these records from
the VDEF request queue using the instructions in the VDEF Install
guide section 6-7.
If you are getting a nuisance number of the false alarm alerts (I'll
let you decide how many is a nuisance to you), I can send you a
preliminary test version of the VDEFCONT program that is creating
these false alarms.

Subject: Remedy ticket 140255 allergy messages not being sent
Importance: High
David, I update the Remedy with this information.
Here's what you should do about your site not sending allergy message since 04/12.
Look at ^XTMP("GMRAHDR","TASK"). If it doesn't exist or has nil data,
then your site must have a different cause for the allergy messages
not being sent to HDR. That will have to be looked at. However, I
strongly suspect that it will have a task number in it. Check the
task number and see if it's still an running task. Again I suspect
that it won't be a valid task.
If this is true, then from the programmer prompt, run this
program: DQ^GMRAHDR
This will start the GMRAHDR process and it will send the queued
up allergy trigger requests to the VDEF queuing API. VDEF will
immediately start processing them and sending them to the HDR.

Gardner, Jeff 727-399-3337
Please read the document I attached for a complete analysis of the problem and the solution.
The short list of why and what to do:
PROBLEM: Site has not sent any allergy HL7 messages to HDR since 04/12/06
CAUSE: ^XTMP("GMRAHDR","TASK") has an invalid task number in it which
prevents GMRAHDR from sending the trigger requests to the VDEF API
^XTMP("GMRAHDR","TASK")=8134261 this is a task that prematurely stopped
SOLUTION: Run DQ^GMRAHDR - this starts the sending of the queued up trigger
requests to the VDEF queueing API
When you have a site that is sending all other HL7 messages from VDEF but no
allergy message, look at ^XTMP("GMRAHDR","TASK").
Normally it is nil after successfully sending all of the trigger requests in
the allergy queue. If it has a task number in it, check to see if the task is still running.
If the task number is invalid or is a task that has stopped prematurely,
start the process manually by running DQ^GMRAHDR from the programmer prompt.
Please assign this ticket to Dave Naber so he can follow up with the site.
I don't have access to San Antonio so I haven't fixed the problem there.
I'll E-mail the site with the instructions to start the process of ^XTMP ("GMRAHDR")

---------------------- --------- --------
---> See Remedy ticket # 134372
Per the above Remedy ticket, your site transmitted VDEF messages to HDR for
all applications other than Allergy (Adverse Reaction Tracking), today
(as of 12 noon central time).
HDR has not received Adverse Reaction data from you site since 04-12-06.
This may be the result of no Adverse Reaction data being entered into VistA.
However, would you please do the following:
1. Review the VDEFVIE2 link using the Systems Link Monitor.
2. Please note the STATE field for the logical link.
a. If it is SHUTDOWN or Halting.
Restart the link using the Start/Stop Links option.
Confirm that the link goes to either Enabled, Inactive,
or IDLE state -
or that you see MESSAGES TO SEND
or MESSAGES SENT incrementing.
b. If the counter fields are not changing and yet the MESSAGES TO
SEND is greater than the MESSAGES SENT, there a message may
be stuck. Please contact me.
c. You can safely ignore the disturbing-looking "Rd-err" and "Wr- err"
3. Use the Link Edit option to confirm that VDEFVIE1, VDEFVIE2, & VDEFVIE3 have AUTOSTART: Enabled.
4. Please reply either in the Remedy ticket or in a reply to this email. Thanks.

The allergy events that are supposed to be turned on are ALGY and ADAS. ADRA has not yet been turned on.
Start by checking the VDEF Custodial package status. It must be ACTIVE. Then check the status of the ORU VDEF Event APIs. They also should be active

Inactivating a custodial package has a significant effect on the
synchronization of VistA and remote system(s). All VDEF requests for HL7
messages associated with this custodial package made while the package is
inactivated will be PERMANENTLY lost. Make sure you really want to turn
this custodial package off.

CHOOSE 1-3:
If these are all active, then check the VDEF Status display once again. All the counters except for Queued Up should be zero.
If VDEF status is OK, do a message search using the HL7 options:
Message Management Options
LOG View Transmission Log (TCP only)
Take all the defaults EXCEPT put in VDEFVIE2 at the "Select Logical Link for Report
When the display comes up you should have some ALGY and ADAS message types in the list.
If there are allergy messages in today's HL7 queue that have been sent out over the link, then I would ask the IE team to take a look. But, all VDEFVIEn logical links connect to the same listener socket on the IE so it's hard to imagine that if the VDEFVIE1 and VDEFVIE3 links are transmitting to the IE that VDEFVIE2 would not be able to.
If there aren't any ALGY or ADAS messages in the HL7 queue and VDEF is not backed up, then the allergy domain is not sending any to VDEF. Use %G and look at the global ^XTMP ("GMRAHDR",)
It's possible that the task sends the allergy triggers to VDEF is backed up.
Please paste all the responses to my requests back into this ticket. Thanks.
That's all I can think of right now.

Although it's not behind the allergy problem, Linda, I sent you the PackMan message ZZVDEFU6 - VDEF UTILITY [#42600748] on FORUM. The description and run instructions are in the message. It will list any VDEF record that is hung in the Checked Out status or in the Errored Out status.
Linda, run DSPLY^ZZVDEFU6 and send me the results back in OUTLOOK since it's not part of the issue with this ticket. I'll take a look at the records and tell you what to do with them.
As for the lack of allergies being sent to HDR, that is because Task 7245372 is no longer valid and the GMRA process that should be getting tasked off to send the trigger events to VDEF has not run since Feb 24th.
To get those allergies sent to VDEF, you can run DQ^GMRAHDR. BUT DON'T RUN IT YET. Dave Naber will want to look at the information in this ticket about ^XTMP before it gets cleaned out.
Lisa, bring Dave into this ticket and let him decide what he wants to do. He has had this problem at another site and it had something to do with Cache I think.

Find out which file it belongs (File #200, 4, 355.93).
-Do use the >> W $$QI^XUSNPI(1815) where NPI=1815
If in File #200, you can delete it by using the
Menu Option >> [XUS NPI ENTER NPI FOR PROVIDER]
-delete all the *incorrect* NPI, then re-enter the correct one if *ALL* are incorrect!
Display the global ^VA(200,”NPI42”,NPI, and this would give you the DUZ who owns it! You can
Delete it IF two NPI are assigned to one DUZ.
Delete also if there is duplicate in NPISTATUS x-ref. See below for sample.
^VA(200,4114,"NPISTATUS",0)="^200.042D^3^3"
^VA(200,4114,"NPISTATUS",1,0)="3060915^1^1134223548"

& nbsp; &n bsp; 2)=""
^VA(200,4114,"NPISTATUS","C",1427210772,3)=""
Use fileman and do Inquiry Profile of that provider. See if dup is still there.
---- sample to find where NPI is residing -------------
IF you substitute your NPI to xxx, you will find out where
(which File) it is residing now.
W $$QI^XUSNPI(xxxx)
Where xxxx=NPI
Sample:
PTH>W $$QI^XUSNPI(1811947815)
Organization_ID^646^3070108^Active;
Analysis:
From this, we could tell that the NPI is under Institution File (See below
explanation). And note that IEN is 646.
Files that would tell you where that NPI already exist:
File #200 >> Individual_ID ^VA (200
File #355.93 >> Non_VA_Provider_ID ^IBA(355.93
File #4 >> Organization_ID ^DIC(4,"
>> Note: NPI can exist on both File #200 and 355.93! But not in File #4 and 355.93!!

Subject: RE: Correcting NPI (Ann Arbor) --
I ended up tracing the code. The NPI field runs an input transform: K:'($ $CHKDGT^XUSNPIE1 (X,DA,"Individual_ID")) X
I looked at the code in ^XUSNPIE1 and it makes a function call to $ $QI^XUSNPI.
At line label QI+19 it sets XUSX equal to the cross reference:
^VA(200,”NPI42”,{npi #},{duz})
^VA(200,”NPI42”,NPI, )
I looked and there was still an old cross reference there for the bad entry.
I killed off that node. This fixed the problem.

Go in an enter a fake number for Stephanie Duke then go back in and mark it as entered in error.
This should bring up the previous number (the one you are trying to assign to Kenneth Wright)
then mark it as entered in error too. This should allow you to assign it to Mr. Wright.
1. Enter a fake NPI (I just entered 10 digits until one passed the check)
Select Provider: `306 SMITH,JOE 111 PHYSICIAN
Provider: SMITH,JOE XXX-XX-5858 DOB: 02/24/1946
Enter NPI (10 digits): 9898653214 < ===== Enter in a fake NPI
Please re-enter NPI : 9898653214
For provider SMITH,JOE (who requires an NPI), the NPI 9898653214
was saved to VistA successfully.
2. Go back in and delete the NPI marking it in ERROR
Select Provider: SMITH,JOE 111 PHYSICIAN
This provider already has an NPI value (9898653214) entered.

------- -Reason why any changes needed approval------------
. There is a official procedure to follow whenever there is a Name or Address
change or any changes in the Institution File. Institution File is managed by my team and yet,
*we are not authorized* to change any information until the 104 FSC office (Financial Service Center
in Austin) has an approval for that change.
Therefore, if there are any changes needed, the site would send the Memo (with their VAMC Director’s
signature) and send it to 10N office, then, 10N office would send it to 104 FSC office for
final approval. Once approved, 104 FSC office notifies different groups including the site that
requested it and my team. Then, that would the time I am permitted to make the change in our
National Institution File and then, populating that change to *all* the sites so we are *all* synchronized.
Once again, my team, can not change address until its been approved by 104 FSC office.
The reason they don’t want any sites changing any information is that IF different sites change their information,
the Institution File wont be synchronized with each other. They have decided to have one National Inst File and
changes can ONLY be made here and then, populating it. Our goal is to have synchronized Institution file.
Hope this information helps.

Description:
One of our dentists has a hyphenated name (Mohammadi-Araghi). When the
Dental Record Manager attempts to locate him in the Denal Provider file,
it apparently looks only for Mahammadi. Thus, there is no match and he is
denied access to DRM. In the test account, I changed his name in the New
Person file simply to Mohammadi and updated the Dental Provider file to
the same name. At that point he was able to log onto DRM. I've looked
in the Production account, and CPRS shows his name as Mohammadi- Araghi on
the top line (when he's in the GUI).
Interestingly, one of our technicians also has a hyphenated
name (Kerns-Lowman) and I'm told she could log into DRM all right! She is
at one of our COBC's today and so I can't check this out.
Resolution:
Notes:
(1) Jul 11, 2001@10:45:16 CREEDON,HUGH
Sorry--but it occurs to me that this may be going to the DSS that is
Decision Support System. I need it to go to the DSS that is Document
Storage System--which supports the Dental Record Manager software. If so,
could you please re-direct it? Thanks. It should go eitehr to CPRS or
Document Storage Systems DRM.
(2) Jul 11, 2001@11:23:13 COOPER,SAMANTHA
Per note #1, changing module to Dental and removing my
name as specialist.
(3) Jul 12, 2001@16:43:44 CARCHIA,CARL A
I will try to duplicate this problem. I don't do anything special with
provider name, but I never tested for this condition.
(4) Jul 13, 2001@16:29:38 CARCHIA,CARL A
I created a long hyphenated name "DSSTESTING-LONGHYPHINATED,DSS" and was
able to logon to DRM. Did you change Mohammadi back to Mohammadi- Araghi
and try to logon to DRM. There is also the possibility of the B-Index
being corrupted. I may get fixed by the name changing.
Select DENTAL PROVIDER NAME: DSSTESTING-LONGHYPHINATED,DSS
DENTAL PROVIDER NUMBER: 9997//
DRM actually uses the DFN and only displays the Name. I am going to change
the code in DRM to only use the IFN and not the Provider Name for the
checks in 220.5.
(5) Jul 16, 2001@15:42:03 CREEDON,HUGH
Well, he still can't log on in the production account. Any suggestion as
to how to proceed at this point??
(6) Jul 16, 2001@17:10:13 CREEDON,HUGH
Rob Silerman has looked at this user's Person File and found what appears
as a duplicate entry. He has removed this entry and I have asked Dr.
Mohammadi to log into DSS DRM. When he attempted to do so, he received the
following error message (after about a two minute delay):
"Could not determine if Moahammadi-Araghi,Mohammad H DSS is not a
registered dental provider within file 220.5. Termiating application."
The message given to someone not in the dental provider file is simply
"Smith,John is not in the dental provider file"--and it delivers this
message rather swiftly.
Don't know if this sheds any light on the subject. He is, by the way, in
the dental provider file and has the correct secondary option.

I will check this message out.
(8) Jul 26, 2001@10:03:33 CREEDON,HUGH
I changed Dr. Mohammadi-Araghi's name (as suggested) in the 200 file,
removing the -Araghi and then re-inserting it. This has not helped and he
still can't log on. Any other suggestions? He, by the way, would not mind
if we simply dropped the Araghi from his name--says he never uses it and
no one knows him by anything other than Mohammadi.

Hugh gave me access to his system today. The problem is a Fileman
problem. I have duplicated the problem on another system. But that
system is not fully patched with latest DI patches. Once patched will
test again before referring to Fileman. DI*22.0*78 patches routines
involved with this problem.
Scenario:
1. Have a file whose .01 field is a pointer file 200 (in this case file
220.5)
2. Have a user use name exceeds 30 characters (new person file allows up
to 35 characters). User in question last, first m deg was 31 characters.
Note: the B index on file 200 is now a new style xref that uses all 35
characters in the B index subscript.
3. Then do Fileman component call from GUI sending the entire person's
name to do a lookup on (all 31 characters)
a. I ran FIND1C^DDR2 which is where DDR FIND1 RPC calls - you get an
error
%DSM-E-UNDEF, undefined variable DINDEX(1,0,1)
-DSM-I-ECODE, MUMPS error code: M6
%DSM-I-ATLABEL, S2+7^DICL2:1 . X DISCREEN(DISUB,DISCR2) S DISKIP='$T
4. I will test further on another system to confirm
5. Temporary solution, edit user's names in file 200 so that they do not
exceed 30 characters
6. Results from another system (not Hines)
a. W NAME,!,$P(^VA(200,1000007055,0),U),!,$L(NAME)
SMITHJONES-COVINGTON,CYNTHIA C DDS
SMITHJONES-COVINGTON,CYNTHIA C DDS
34
b. K RET,ERR W $$FIND1^DIC(220.5,,,NAME,,,"ERR")
%DSM-E-UNDEF, undefined variable DINDEX(1,0,1)
-DSM-I-ECODE, MUMPS error code: M6
%DSM-I-ATLABEL, S2+7^DICL2:1 . X DISCREEN(DISUB,DISCR2)
S DISKIP='$T
c. K ERR W $$FIND1^DIC(220.5,,,$P(NAME,U,1,2),,,"ERR")
K ERR W $$FIND1^DIC(220.5,,,$P(NAME,U,1),,,"ERR")
You get the same error as in 6.b
Notice that the DBS call does not trap the error and return error
in ERR but instead errors out which it should not do.
d. Edit name in file 200 to bring name down to 30 characters
W !,$P(^VA(200,1000007055,0),U) S NAME=$P(^(0),U)
SMITHJONES-COVINGTON,CYNTHIA C
e. K ERR W $$FIND1^DIC(220.5,,,NAME,,,"ERR")

f. In this account (I do not have the status of Fileman patches at
Hines), all FM patches are installed except for the following:

this problem. After patches are loaded will try again.

Problem has been resolved. The dentist's last name was shortened from
Mohammadi-Aranghi to Mohammadi. Change was reflected in the Dental
Provider file and all is working OK now.
(11) Aug 02, 2001@10:42:31 MCPHELAN,STEVEN
I will run the tests I mentioned today to see if this needs to changed to
Fileman.
(12) Aug 05, 2001@11:48:56 MCPHELAN,STEVEN
I have reproduced the problem as outlined in response #9, step #6 on a
fully patched system. So I am changing this to Fileman.
(13) Aug 06, 2001@09:25:08 ALBARRAN,JAVIER
Steve had replicated the problem so I am moving the NOIS to SD&D. Thanks
Steve for trouble shooting. I thought you retired.
(14) Aug 06, 2001@15:02:04 ORMSBY,SKIP
Thanks everyone for your trouble shooting efforts. What I have found so
far is that the classic call works. And if you change the length of the
"B" index on the .01 field of the file that is doing the pointing then all
is find with the two DBS calls. Both FIND1^DIC and FIND^DIC error out.
I of course will keep trouble shooting but did want to let you know that I
could replicate the error.
(15) Aug 17, 2001@14:58:35 ORMSBY,SKIP
Sorry about not responding sooner but had a few emergency calls to take
care of. The problem stems not from the hypen in the name, but the
different lengths of the index of the Pointing File verses the file that
is being Pointed To as Steve indicates in Note #9. I have a fix ready to
be moved forward. Is the site or anyone willing to test?
(16) Aug 29, 2001@12:27:30 MCPHELAN,STEVEN
Skip I will test the patch for you.
(17) Sep 04, 2001@09:29:12 ORMSBY,SKIP
Thanks Steve for volunteering. I'll start doing the patch process. To
what address would you like me to send it to?
(18) Sep 04, 2001@12:18:43 BEUSCHEL,GARY
I thought you were retiring, Steve?
(19) Sep 04, 2001@15:48:25 MCPHELAN,STEVEN
I did retire. The early out offer to start collecting CSRS retirement was
too good to pass up. I went to work for Document Storage Systems, Inc.
Right now their only customer to speak of is the VA.
(20) Sep 04, 2001@16:29:51 BEUSCHEL,GARY
Cool!
(21) Sep 06, 2001@08:42:24 ORMSBY,SKIP
Forwarded the POC and test v1 to Steve and Hugh. Thanks again folks for
being willing to test this patch.
